You've got an M-spike of 0.2 g/dL and your serum immunofixation identifies the M-spike as being IgG kappa. Someone who does not have multiple myeloma or MGUS (monoclonal gammopathy of undetermined significance) does not have an M-spike, and normally would not have the serum immunofixation pick up any monoclonal protein.

If you find a monoclonal immunoglobulin on an SPEP (or UPEP), you can characterize it using immunoelectrophoresis or immunofixation.
